- DMV Waiver - (mailed to student)
- Medical Form - (mailed to student)
- New York State Drivers License - needed to make copies
- New York State Motorcycle Permit - needed to make copies
- Deposit - must be made once the class dates are given
- Full payment - All payments must be made before class starts
